A bantamweight and featherweight during his career, O'Keefe's first challenge came in 1911 against world bantamweight champ Johnny Coulon in a non-title bout. On April 25, the two fighters battled to a 10-round draw just two months after Coulon won the title. The following year, O'Keefe fought the great Johnny Dundee twice in the span of four weeks with both bouts ending in no decisions. Later in 1912, O'Keefe also fought featherweight champion Johnny Kilbane in two no decision non-title bouts. In 1913, O'Keefe and Kilbane fought again and Eddie was knocked out in the first-round.

In 1913, O'Keefe continued to fight the top featherweights and battled contender George "K.O" Chaney on Christmas Day; he was knocked out by Chaney in the fifth-round. The two fighters had a rematch two years later and the bout ended in a no decision; in 1918, they fought a third time with Chaney knocking O'Keefe out in the fifth-round. During the mid-1910s, O'Keefe also fought future lightweight contender Lew Tendler twice (both no decisions), former bantamweight champion Kid Williams (no decision), and future champ Joe Lynch (knocked out in the fourth-round).
