At the 1912 Games, the first to include women's swimming, Milch competed in both individual and team events. In the 100-meter freestyle, she finished third in her preliminary heat with a time of 1:37.0 and did not advance to the semifinals. She swam the second leg of the 4x100-meter freestyle relay, with Jewish teammates Margarete Adler, and Josephine Sticker. The Austrian team finished in third place with a time of 6:17.0 to take the bronze medal (Great Britain won the gold in 5:52.8).

Career Highlights:
Milch was one of Austria's top swimmers in the 1900s and 1910s. She was the Austrian National champion in the 100-meter freestyle (1907-1912), the 100-meter breaststroke (1911), and the 100-meter backstroke (1912-13).
